If you want the icon to appear smaller than 1-by-1 inch scale it to fit the size of the box drawn with the button tool. Delete un-embed or remove the Adobe Acrobat icon from the Start menu the desktop and if available the taskbar. Now open the PDF in IE and all the bookmarks are available.
